TS3 DNS Manage

Hey guys. I know this thread is posted daily and I read a lot about it but still can't get this stuff to work.
I have a domain (titanmc.hu) and a Teamspeak 3 server (37.221.209.209:10011)

I've got a DNS manager provided by my hosting.
I heard that i have to use SRV records,but i can't figure out the right solution.
It asks for :
Name
Type: (SRV)
TTL
Priority
Weight
Port
Target

Any help appreciated!
